Microsoft Forms is now UNC Health's preferred survey application, replacing SelectSurvey.NET (Survey Tool).

Respondents: If you received a survey link via email, follow-up with the email sender. If you accessed a survey link from a SharePoint site or website, submit an incident survey request ticket for further assistance. Be sure to include the survey name and where the link was accessed so we can better assist you.
